The album certainly doesn’t need an introduction, nor does the cover.
I will say only this: the idea of Kurt Cobain represented on the cover was to summarize what society expects from each human being, that is, to spend their entire life chasing money, from birth to the grave.
The child in question, Spencer Elden, has since grown up, sued the band for sexual exploitation and child pornography, citing emotional distress, loss of earning capacity, as well as "loss of enjoyment of life": draw your own conclusions...
