STUFFED MUSSELS. INGREDIENTS: 1 kg of mussels, 30 g of salted capers, 50 g of breadcrumbs, 30 g of pecorino romano cheese, 400 g of tomato pulp, 2 cloves of garlic, 1 teaspoon of oregano, 1 egg, 1/2 cup of dry white wine, a bunch of parsley, extra virgin olive oil, salt. PREPARATION: The mussels: soak them in cold water for about 2 hours, then clean them by scraping with a knife and place them in a saucepan with the peeled garlic and wine, until they have all opened, then remove from heat and drain, reserving the cooking liquid which you will filter with a fine sieve. Continue: shelling 15 of them, then blend them in a mixer with half of the parsley leaves, the breadcrumbs, the egg, the pecorino, the capers, and the oregano. In a pan: heat 4 tablespoons of oil, add the tomato pulp and the filtered liquid from the mussels, cover with a lid and cook for about 15 minutes. The remaining mussels: stuff them with the filling and tie them with a thick string to keep them closed, place them in the tomato sauce you just prepared, add a ladle of boiling water and let cook for about 20 minutes. To serve: sprinkle the mussels with the remaining chopped parsley. DONE AND READY TO EAT!

The perianal skin tends to itch. Anal itching has many causes: (1) dermatological disorders (e.g., psoriasis and atopic dermatitis); (2) allergic reactions (e.g., contact dermatitis due to local anesthetics, like preparations ending in "-caine," various ointments, or other aromatic chemicals used in soaps), eczema following the ingestion of certain foods (especially spices, citrus fruits, vitamin C tablets, coffee, beer, and cola), although it is doubtful that a true allergy is the causal agent; (3) microorganisms such as fungi (e.g., dermatophytosis and candidiasis) and bacteria (secondary infection due to scratching); (4) parasites (intestinal worms and, less commonly, scabies and pediculosis); (5) the administration of oral antibiotics (especially tetracyclines); (6) pathological processes such as systemic disorders (e.g., diabetes mellitus, liver diseases), proctological diseases (e.g., fissures, cryptitis, secreting fistulas), and neoplasms (e.g., Bowen's disease, extramammary Paget's disease); (7) hygiene, either poor, with irritating residues of feces, or too meticulous, with excessive use of soap and detergents; (8) heat and hyperhidrosis linked to the use of overly tight tights, tight pants, and too many bedcovers, obesity, and climate; and (9) the psychogenic response. Hemorrhoids usually do not cause anal itching. Skin changes can be characteristic (e.g., insensitivity, thickening) or minimal and may be masked by excoriations due to scratching and secondary infection. A scrape of the local skin may help uncover fungal infections, and a stool sample may help identify parasites. THERAPY: Since certain foods can cause anal itching, the effect of eliminating them from the diet should be assessed. Clothing should be loose and non-tightening, and bed covers should be light. After each bowel movement, the perianal area should be cleaned with absorbent cotton dampened with water. Frequent and free dusting of non-medicated talcum powder helps combat moisture. 1% hydrocortisone acetate in the form of an emulsion, applied sparingly qid, is usually very effective. Topical antifungals (e.g., amphotericin B, ciclopiroxolamine cream at 1%) can be used. Systemic causes and parasitic infestations must be treated specifically. Biopsies of refractory lesions may reveal malignant neoplasia. Radiation therapy and surgery or injections to achieve permanent local anesthesia are rarely indicated.
