It refers to drug addiction, at least that's how I've always interpreted it, and all the people I know do too; Hetfield also insinuates this in the MTV Icon interviews.

So I'm in agreement, Kirk wrote solos that the entire Imola racetrack sang in every single note, Kirk in the Olympus, I repeat that Dime is my favorite, but speaking of technique it's completely normal, since personal musical taste is what’s at stake. Friedman is a monster, you're right Cliff, but he’s more technical than "musical" in my opinion.

Great angel, you mentioned parts that I also like a hell of a lot. Well, I wouldn’t know what to choose for individual, Mentally Blind is my absolute favorite piece by Death even though it’s not the most complex nor the most musically appealing.

But City Hunter wasn’t the one with the dragonflies passing in front of the screen from time to time. Hey, I’ve always liked this CD; I don’t know where you got this idea, which is also well defended to make me look like an idiot if I reply. But whatever, if I had something to say about the Sadists, I must have had something to say about ...Crust, which I actually like to a certain extent. Regarding Tribe, I can say that their synths annoy me; charm or no charm, if they had put those nice, shabby effects from real synths, it would have surely been an engaging work.
