The Stener lesion is an injury of the ulnar collateral ligament, which may be associated with the detachment of a bony fragment. It also occurs in skiing when, during a fall, we instinctively tend to open our hands and force the thumb to move outward. This sudden strain can cause partial tears of the ligament or its complete rupture.

It does not only affect skiers; it can also involve motorcyclists when, following a fall, the thumb accidentally gets trapped in the handlebars. In cases of partial tear, the thumb is immobilized for three to four weeks. In cases of rupture, surgical intervention is required to repair the ligament: an incision is made at the site of instability, the damaged ligament is identified, and it is reinserted into its position.

If a bony fragment becomes detached from the base of the first phalanx of the thumb, small anchoring systems embedded in the bone and sutures are used to trap the bony fragment and reposition it. Post-surgery, immobilization continues for three weeks, after which rehabilitation begins with a splint, usually a wrist brace with the first finger included.

Often, we specialists see the fracture even two or three weeks after the injury. This happens because the patient underestimates the trauma or because, for instance, during the assessment at the Emergency Room, the injury is not correctly recognized. The more timely and accurate the diagnosis of the injury, the sooner it can be treated properly. Most people are generally unaware that they have this injury, and after some time, the pain subsides, but instability remains: the thumb will start to cause problems, with the risk of early osteoarthritis and accelerated wear of the cartilage components.
