GI is punk '77 dry, are we kidding? The Ramones are more like proto-punk; the punk stylistic elements we recognize largely come from the Pistols. If anything, the thing closest to punk before Nevermind the Bollocks was Blank Generation by Richard Hell.

There are tons of hardcore bands, my favorites are Minor Threat, Operation Ivy, Dead Kennedys, Husker Du, and Minutemen - for example, I am allergic to Black Flag, you see...
The problem is that hardcore is often mixed with other genres, Minutemen are hardcore and funky, Dead Kennedys are hardcore and country, Husker Du are hardcore and psych, the line separating it from post-* is very thin.
Sure, Fugazi are not hardcore; the moment you say "refined," you are out of hardcore :)

The Jesus Lizard are great, Goat in particular, but they don't have much to do with it. Back then, the MT were very well-known, at least as much as the Bad Brains, but while the Bad Brains were a "generic" band, the MT had a loyal following that, incredible as it may seem, shared their ideas. Musically, the MT were the first to go that fast; you can't imagine what the concerts were like :)
