Thanks to everyone for their comments, but in the end, for me, the concept of extremism remains subjective. For example, I find this album to be among the most extreme I’ve listened to, but it depends on the parameters used to classify extremism. For instance, awful production can be both an advantage and a disadvantage. I like Heartwork as an album, even though I like it less than the earlier ones, although I recognize it has had a decisive historical importance for the so-called Melodic Death.
